The Weight Loss Surgery Team

Milford Regional's Fresh Start Program is committed to the success of our patients. We offer a multidisciplinary team approach that is comprehensive and personalized to help you achieve your weight loss goals.

Each member of our team; physicians, dietitians, behavioral therapists, nurse, exercise specialists, administrative assistant, and nurse practitioners all bring a unique set of skills to meet the individual needs of our patients. With the help of every member of our team, and ongoing commitment from our patients, small, practical steps and goals can lead to long-lasting, healthy weight loss.

Program Coordinator
Edlaine Machado Program CoordinatorEdlaine Machado, RN, BSN earned her Bachelor of Science degree in nursing from Rhode Island College, Providence, RI.  She is bilingual in Portuguese, highly proficient in Spanish and is BLS and ACLS certified.

Edlaine has been a nurse at Milford Regional for the last eight years caring for patients on our medical surgical telemetry unit and progressive care unit.  She has also served as a resource nurse for her nurse colleagues in the provision of care for the bariatric population including the creation of a dietary documentation tool and assessment guide.

Prior to joining Milford Regional, Edlaine worked at Beaumont Rehabilitation and Skilled Nursing where she was a charge nurse as well as a nurse mentor providing educational training for new graduate nurses.

Donald Czerniach, MD, FACS

Donald Czerniach, MD

Dr. Czerniach received his degree in medicine from the University of Medicine and Dentistry of New Jersey, Newark, NJ in 1995. He performed his surgical residency at George Washington University Hospital, Washington DC where he was chief resident. Continuing his training at the University of Massachusetts Memorial Medical Center, Dr. Czerniach completed a research fellowship and clinical fellowship in advanced minimally invasive surgery.

Dr. Czerniach is an Assistant Professor of Surgery at the University of Massachusetts Medical School and he is the Director of Minimally Invasive Surgery at UMass Memorial Medical Center.

Matthew C. Rade, MD

Matthew Rade, MD

Dr. Rade received his degree in medicine from the University of Buffalo School of Medicine and Biomedical Sciences in 2015. He performed his surgical residency at Lahey Hospital and Medical Center in Burlington, and followed by a fellowship in minimally invasive surgery at Baystate Medical Center in Springfield.

James C. Senturk, MD, PhD, FACS

James C. Senturk, MD

Dr. Senturk received his degree in medicine from the Icahn School of Medicine at Mount Sinai in New York in 2015. He performed his surgical residency at Brigham and Women's Hospital in Boston and continued  his training with a fellowship in advanced gastrointestinal and minimally invasive surgery at New York-Presbyterian Weill Cornell Medical College in New York.

 

 

 

 


Pre-Registration

Patients should call to pre-register up to two weeks in advance for any scheduled surgery and out-patient appointment including most diagnostic tests, lab work and pre-operative testing. This eliminates the need to wait for an admissions representative upon arrival. Patients can go directly to their point of service. Admissions representatives are available to take calls from 7 a.m. to 5 p.m., Monday through Friday.
