Milford Regional's Care Management Program demonstrates our commitment to your complete treatment and recovery care. From your admission to your post-hospitalization recovery, our Care Management Team is here to coordinate all your Medical Center services and assist you with any arrangements needed following your discharge. Your total care is our complete concern.

Milford Regional Care Management Team

As part of our ongoing commitment to total quality care for our patients, Milford Regional has implemented the Care Management Program, a team approach to your care that combines the resources of utilization review/case management and clinical social work.

Nurse and patient holding handsA nurse case manager works with your physician and other team members to coordinate all the services provided to you during your stay at Milford Regional. The nurse case manager will also access your need for physical care after you leave our facility. A social worker is also available to help you meet your emotional needs along with any financial and placement concerns you may have.

This team approach will help to ensure your smooth transition from admission to discharge, all the while providing you with exceptional healthcare services. The benefits of this program include having your concerns and questions addressed quickly and efficiently, decreasing fragmentation of your healthcare services to provide a smooth course of treatment and coordinating an effective, safe and timely discharge plan based on your individual needs.

Post-Hospitalization Options

Your doctor may decide that after discharge from the hospital you need more nursing care or rehabilitation services. This care may be needed for a short time or a longer period and can be given in an extended care facility (skilled nursing facility or nursing home), about which you can receive information through Care Management. Your doctor may also recommend assisted living – a supportive housing option for elders and others who are unable to live on their own, yet want to remain as independent as possible – or home healthcare services, which can be provided by a number of agencies in the area. Your Care Management Team can explain the differences in the above kinds of post-hospitalization care, help guide you and your family through the decision-making process, and answer questions you may have.
